Here is a list of common errors you might see when you try to send a claim to Claim.MD using the Clearinghouse Direct billing method.
- Failed to submit the claim.due to: 'undefined method 'gsub' for nil:NilClass
- Possible cause: Missing diagnosis code on an invoice or client note.
- Possible cause: Missing date of birth.
- If you checked the box on the client's CMS-1500 and Clearinghouse Settings area for Signature on File, make sure you specify a date. This can't be left blank.
- Failed to submit the claim. due to: 'for segment NM1, elements 9 must be present when elements 8 are present'
- Missing Insured's ID number and/or the insured's relationship on Client's Insurance billing settings page.
- Failed to submit the claim. due to: 'required element N301 is blank'
- Check to see if the client's diagnosis is present for the date of service you're billing for.
- Check your office locations to make sure the address, city, state and postal code are present
- Check to see if the client's address, city, state and postal code are set-up on their client information page.
- The claims address is missing from the insurance provider's settings page.
- Failed to submit the claim. due to: 'require element NM109 is blank'
- Possible cause: Payer ID is missing from an insurance provider's settings
- Failed to submit the claim. due to: invalid DTP segment
- At the bottom of a client's billing settings page there is an area called CMS-1500 and Clearinghouse Settings. This error is caused by filling in the referring provider and entering the start date of the current illness. This is only required if the initial medical service or visit is in response to a medical emergency and the date must be different than the date of service. To fix this, remove the start date of the current illness.
